Each patient with progressive or advanced CKD or on renal replacement therapy should have an individualized ESRD Vascular Access Life Plan. Nephrologists, vascular access surgeons, dialysis nurses, vascular interventionalists and ultrasonographers form a team to optimize timing and success of vascular access placement. Early detection of immature fistulas will lead to timely intervention and result in decreased TDC placement and time on HD with a TDC.

Learning Objectives

After participation in this activity, learners will be better able to:

  • Recognize the challenges that the elderly ESRD patient poses with regards to vascular access
  • Select the appropriate vascular access for hemodialysis in the elderly patient
  • Diagnose and treat immature vascular accesses and thereby decreased HD via TDCs
