Improving Access to Health Data for People with CKD and Their Healthcare Team

People with chronic kidney disease (CKD) undergo frequent care transitions (e.g., hospital to home) and require support from a variety of providers and services across multiple settings, generating challenges with care coordination and communication that may result in avoidable adverse events, diagnostic delays, increased cost, poor care quality and safety risks.
 
Electronic (e)-care plans enable sharing of key patient data, supporting care coordination, communication, and collaboration over time across the care team (e.g., patients, caregivers, primary care providers, nephrologists and other specialists, social services providers, etc.) and settings via an interoperable platform (e.g., health information exchanges).

This webinar will acknowledge existing care challenges related to poor availability and interoperability of data, review the evidence base supporting development of e-care plans, present efforts of the NKDEP CKD Care Plan Working Group, and discuss opportunities to improve e-care planning efforts in CKD.
